Easily reproducible and likely an OCCT related issue, but I would concur that this is fundamentally poor behavior and is likely related to OCCT's behavior regarding fillet/chamfer overlap/consumption of faces/edges. The first (0.01mm) fillet is to modify the edges of the next fillet. Nice workaround, such a tiny fillet won't even be visible.
