Something like this? Small threads are pseudo trapezoidal - large is a 3 start flute. Should work for both a mill 4th axis and lathe, with or without a forming tool. Of course there's my +1 on this, but after going over the examples you already have, not sure what could be added. Maybe some "basic" mold shapes for the dropcutter / waterline? :?

Attaching your sample file(s) would be helpful, but from looking at your screen caps, look closer at your Final Depth. Is 0.000 at the top of the part?

Sorry about not responding sooner.. thought someone with more time in on Path would of picked this up. My bad.
