You use an outdated FreeCAD version, but it happens in 0.19 too, and it is strange. kurt2me uses Part workbench, and there he would need two lofts. However, these examples are pretty convincing to give PartDesign a try.

Very interesting reading, although the ISO standard mentioned at the end of the introduction are rather outdated. At least the outdated book I have mentiones newer versions.

Do you have access to the current ISO standards?

So then it's kisolre's guess: In a Part loft inner holes are not allowed and sketches containing such holes don't show up in the list. You have to create an outer and an inner loft and subtract them.

That is well known, many applications show this behaviour. The first time you have to open it by right click + open, and then you can confirm to open anyway.
