Clicking 'planar' when creating the Face seems to make a good face, but it still gives the crumpled look when lofted. Maybe I can place a circle on that edge and loft from that.

I increased the height of the loft by 1mm, and the cut worked as expected. Thanks for the help.

Do you mean I have somehow inadvertently created a superfluous vertex? I recreated my model from scratch and still have the same error.

Perhaps "sketch" should extend only as far as the present Loft, i.e. as far as sketch001. Then add an additional sketch for the rectangle between sketch001 and sketch003. Then you could loft from the new rectangle to sketch004.

With your file tube.fcstd open in the Part Design Workbench, rotate the view to show the bottom of the pad. Click on the bottom face of the pad, then click on 'Map a Sketch to a Face'.
