It could be a driver issue if you're on Windows using 0.18.4. This is why it's important to provide the FreeCAD information: help -> about -> copy to clipboard. If this is the case you can update to the latest 0.19 and see if that solves it.

You could attach the subtractive cylinder to one of the circles on the top face of the pipe, and then adjust the rotation to 90 degrees about x or y. I was able to get a valid object, but it took some doing and is far from ideal. The result is not fully parametric. Install OpenSCAD if you don't already have it. Then take the sketch on surface object to OpenSCAD workbench and perform the boolean hull operation on it. It gives you this mesh object:...

Thanks for the extra input. It's true this applies to the linear transformation too. I could not understand the "apply axial" button at first. What axis is it applying to ? A better label may be " apply consecutive rotations". If I get a chance , I'll submit a PR for that. It applies not only to consecutive rotations, but also to consecutive transformations. Perhaps, "apply consecutive changes" would be better. I don't know, but I think "changes" to "rotations" might ...
