You should use 0.19 if you want to scale a Mesh. It wasn't present in 0.18. I think its been improved in 0.19, but it is there in 0.18.4, I only tested on a simple Mesh cube, so more complex meshes may have issues that I did not get in my very simplistic test. You can easily attach the second cylinder to the first, either by editing the second cylinder's MapMode or by using Menu->Part->Attachment. After that the Placement is relative to the Attachment. Since the piece is appropriately designed as a .STL file I will download a separate application, thanks!
