In your video you are attaching to a Vertex - it has only position but no orientation. So the only meaningful attachment offset to only a Vertex is Translate origin. All angle values are ignored. Try attaching to an edge...

Without any relevant info no way to help. Please check Important and especially point 5, 6.

Create a full topview sketch1, pad. Pocket the tail. Create a sketch at pad height, carboncopy sketch1 as construction geometry, create edge between A and B. Map sketch to that edge, cut front tip at 105 deg.
