At on time the tool bars I was frequently using had buttons for undoing and redoing the previous action(s). Now they seem to be missing. How can I get them back? In general, how are buttons added to toolbars?

Right click on any toolbar and see if the one named File is checked in the context menu. File is the name of the toolbar with the undo / redo buttons.
