For the first question
Meanwhile, if you check the PolarPatterm of the original topic file (attachment P01) you will see that the occurrences are "3" instead of "4", if you recalculate "PolarPattern", PartDesign will correct it and you will see 3 grooves with error generation in subsequent fillets .

I don't understand why FC doesn't report the recalculation! Maybe a topological renaming bug (?)

Then ask if it is better to do (... ... PolarPattern) 4 or 2, I believe that for modeling purposes the choice of one rather than the other depends on the way the Pocket was created:
- in P01 the Pocket is set through everything, not symmetrical, not inverted; therefore only one groove, so the PolarPattern must have 4 occurrences and 360 ° to obtain the four grooves (optimal and correct solution).
- in the P01-2 the Pocket is set through everything, symmetrical to the plane; therefore two notches, so the PolarPattern must have (minimum) 3 occurrences in 180 ° to obtain the four notches (non-optimal forced solution), instead (minimum) 2 occurrences in 90 ° to obtain the four notches (it would be the optimal and correct solution ).

As regards the second question
- Nothing has been changed in the settings of the original sketch, except those relating to the fillets that were inserted later to eliminate the next two filleting phases.

Certainly it would be more logical to set the constraint on the radius.
Generally, when possible, it is always better to set geometric constraints rather than dimensional constraints.
