After installing 0.20 and removing 0.19, the file associations broke. I have tried using the Control Panel to associate the file extension with the FreeCAD.exe in the C:\Program Files\FreeCAD 0.20\bin directory but that did not work. Also when I open freeCad the Start screen used to show the redF and Gaer symbol for each of the historical files. that doesn't happen either.
Do I need to reinstall?

Do you mean the icons of the *.FCStd files or the program itself? Here the program icon now looks like this:
